Before your first appointment, please print and complete the intake form. You also can print a copy of our office policies/procedures along with the privacy form.

Blue Water Serenity Counseling, PLLC, provides a notice of privacy practice policy on site. The practice provides this notice to all clients to comply with the privacy regulations issued by the Department of Health and Human Services.
